Residents from ANY town may attend our clinics and PRE-REGISTRATION is required.

The only insurance accepted is Medicare Part B. For all insurances other than Medicare Part B, including Medicare Advantage plans such as HMOs, PPOs, etc., the cost is $20 (regular vaccine) or $63 (high-dose vaccine). Cash only. Please note, if you are enrolled in a non-Medicare Part B plan (HMO, PPO, etc.) you may wish to receive your vaccine from your primary care physician, where there may be no charge.

Medicare Part B participants must bring their Medicare Part B card in order to bill Medicare. Children ages 3-17 can be vaccinated only if a parent or guardian is present. Some children aged 3-8 may need to receive a second dose. The vaccine for children is preservative-free.

If anyone is in need of a COVID test, you can call Borough Hall for a free test while supplies last.

Dog and Cat Licenses
May be obtained via by mail or at the Borough Hall:
Mon, Wed, Thur & Fri from 8:30am - 4:00pm; 
Tues - 8:30am - 6:00pm.
Licenses run from January 1 to December 31. All pets over the age of six months must obtain a license which requires a current rabies inoculation, good for the entire calendar year, certificate and proof of being spayed/neutered required to avoid additional State fee. The Borough has a "Scoop the Poop" Ordinance and a "Leash Law" for dogs. 

Rabies Clinic
2024 Rabies Clinic is scheduled for Thursday, February 15, 2024 from 6-7pm in the St. Lukes Church Parking Lot.

Marriage License

Marriage license applications are made by appointment only. Please call 201-652-4400, ext. 223 to schedule an appointment within 30 days of your marriage date. 

The cost for a marriage license is $28 payable by check or cash. Credit cards not accepted. You will need to provide the following:
- Two (2)  separate checks
     (1) check for $25.00  and (1) for $3.00 both payable to the Borough of Ho-Ho-Kus 
- Both persons need to bring in a copy of their Driver's License and Birth Certificate
-  A witness over 18 years of age

Well Baby Clinic - There is an interlocal agreement with the Borough of Waldwick that offers a Well Baby Clinic by appointment only on Tuesdays from 9 -12 PM.
